The customer reported the colonovideoscope head had a separate issue.During the initial evaluation of the device it was observed, air/water channel has foreign material.No patient involvement.

Based on the final investigation of the device, the initial evaluation findings were confirmed.A root cause could not be identified.Based on the results of the investigation, it is likely the following led to the malfunction air/water channel foreign material cause could not be established.A root cause could not be identified.Based on the results of the investigation, it is likely the most probable cause of the malfunction could not be established.However, use of products such as simethicone, lubricants that contain silicone can cause deposits of white foreign material and thus are not recommended for use.Olympus will continue to monitor the field performance of this device.Should any additional relevant information become available, a supplemental report will be submitted.
