Additional Manufacturer Narrative
The customer reported a quality concern for potential growth of mold on the chloraprep applicator; the issue did not occur during use and no injury occurred.Sample evaluation found that two photos were provided for evaluation: one chloraprep outside of the pack with a blue stain present on the inside of the lip of the sponge portion and one applicator still present within the kit with the same blue stain at the top of the sponge portion of the chloraprep; finished good production records were reviewed with no nonconformances noted, and the bom was reviewed with no issues identified. Two photos were provided of the chloraprep applicator with blue staining, confirming the reported issue. The investigation was limited due to the absence of a returned complaint sample, and it could not be determined whether the chloraprep applicator was activated prior to placement within the kit or occurred during transit or storage; as a result, a definitive root cause could not be determined at this time, and the complaint was provided to the vendor for further evaluation while the division will monitor this issue for trending purposes. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
