Additional Manufacturer Narrative

An investigation was completed to determine the cause of this reported event. An intuitive surgical, inc.(isi) field service engineer (fse) was dispatched to the customer site to further investigate the reported event.Or staff, contacted tech support mid-procedure to report they had a recoverable error earlier in the procedure resulting in the system restarting on it's own.Prior to calling they restarted the system and were continuing as planned. As the caller was explaining what happened communication errors returned pointing to the blue fiber cable connection to console 1 plugged into the 3rd from the top port on the back of the core.Tse walked the caller through powering the system down and reseated the fiber cables.Caller was not able to perform a hard power cycle and powered the system back on resulting in a error 307 on console 1.Customer elected to undock and finish the case as planned.Field engineer (fe) replaced personality module surgeon console (pmsc) as per isi procedures.System is operational and verified as ready for use.Isi did receive a da vinci product involved with this complaint to perform failure analysis, but evaluation has not been completed as of the date of this report. The complaint was confirmed based on the field evaluation, which indicates that the device may have contributed to the customer reported issue.
