Event or Problem Description
|
|
It was reported that this subcutaneous implantable cardioverter defibrillator (s-icd) system was unable to induce an arrythmia at implant.Imaging was completed, and it appeared the s-icd system was too superior but was unable to be confirmed by the imaging performed at that time. After being discharged after implant, the patient experienced two inappropriate shocks and presented to the emergency room (er).In the er, the patient's device therapy was programmed off.Technical services (ts) reviewed the two shock episodes. The first shock episode was caused by small amplitudes and oversensing of noisy signals. The second episode was caused by small amplitudes and t-wave oversensing. There was no change in rhythm after shock therapy.Ts recommended performing further imaging to confirm system location and determine if a system revision was required. This electrode remains in service. No additional adverse patient effects were reported.
