The customer reported fluid not pushing through tubing and stated there were no issues flushing through the set; the issue was when it was connected, over half did not allow her to flush through once connected, and the clamps were not being used.A sample was received, medline connected the female end of the sample to the male end of an iv administration set, and liquid was able to flush through the item; customer samples were provided to amsino for further evaluation.Based on the complaint details and investigation, medline confirmed the issue.Amsino's investigation determined that the defect originates during the assembly bonding process, where tubing was bonded such that it extended into the inner diameter of the male luer, resulting in interference between the tubing and the internal features of a needle-free site upon connection, which blocked or severely limited fluid flow through the connection; corrective action has been issued to amsino, and the supplier quality team will monitor this issue for trending purposes. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
