Event or Problem Description
The customer returned the gastrointestinal videoscope for a separate issue.No patient involvement.During inspection of the returned device, it was found the air/water port inlet was rusty.
Additional Manufacturer Narrative
The device was returned to olympus for inspection.A root cause could not be identified.Based on the results of the investigation, a probable root cause was traced to a component failure.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
