Event or Problem Description
The customer returned the colonovideoscope for a separate issue.During the device analysis, the service repair technician observed that white foreign objects were present in the air water cylinder and tube.There was no patient involvement.

Based on the final investigation of the device the initial evaluation findings were confirmed.A root cause could not be identified.Based on the results of the investigation, it is likely the following led to the malfunction: cause not established.Olympus will continue to monitor the field performance of this device.Should any additional relevant information become available, a supplemental report will be submitted.
