Additional Manufacturer Narrative

|
The permanent cautery spatula (pcs) instrument has been returned and evaluated by the failure analysis team.Intuitive has received the part associated with this complaint and completed investigations.Fa investigations replicated confirmed the customer reported complaint. The pcs instrument was found to have a broken distal clevis at the ears of the clevis. The broken piece was not returned.Clevis does not show abrasions or scratches on the outer surface.Components adjacent to the broken clevis show damage which may indicate potential misuse.Additionally, the pcs instrument was found to have a broken monopolar yaw pulley at the ceramic insulator interface.Broken pieces are missing as a result of breakage. The components surrounding the break did exhibit damage that would have contributed to the broken yaw pulley. The instrument conductor cap was found to be broken. As a result of the break these components are considered detached fragments. The detached pieces were not returned with the instrument. The instrument was found to have the monopolar conductor wire/dislodged from its original position. The probable root cause is attributed to use conditions.
