The customer stated that ¿the arm does not align with the holes it doesn't fit the frame it snapped off,¿ and ¿no injuries reported.¿ photo/video was provided; a physical sample was not provided, so only a visual review of the defect could be conducted, and inspection records were reviewed with no nonconformances that correspond to the defect reported. The investigation results were confirmed: the product appears to be in used condition as indicated by the wear on the wheels, and the left push handle tubing appears to be fractured directly above the locking mechanism. The root cause of the reported issue cannot be determined at this time; the replacement push handle purchased by the customer is a different style and design than the original push handle, and the currently available replacement parts are not compatible with older model units. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
