It was reported that before a laparoscopic-assisted distal gastrectomy (ladg), the outer seal came off and fell on the floor when the package was opened.The device was not used for the patient.Another device was used to complete the case.There were no adverse consequences to the patient.

(b)(4).Batch # = m93k5n.The analysis results found that the b12srt device was returned in good condition and out of its sterile package.In addition the universal seal was returned not latched to the sleeve.In an attempt to replicate the reported incident, the universal seal latch onto the sleeve assembly.In addition the obturator latched onto the universal seal as well.No seal issues were noted during functional testing.No conclusion could be reached as to what may have caused the reported incident.A batch record review was performed and no anomalies were found during the manufacturing process.
