Results: the penumbra system ace 64 reperfusion catheter (ace 64) was kinked approximately 3.5 and 52.7 cm from the hub; and ovalized approximately 87.0 and 90.5 cm from the hub. Conclusions: evaluation of the returned device revealed it was kinked in two locations on the catheter shaft. This type of damage typically occurs due to improper handling during removal from the packaging. If the ace 64 is manipulated forcefully at an extreme angle during removal from the packaging hoop, damage such as this may occur.Further evaluation revealed the ace 64 was ovalized in two locations. This damage likely occurred due to forceful gripping or pinching of the ace 64 during removal from the packaging.Ace 64 devices are 100% visually evaluated during in-process inspection. The manufacturing records for this lot were reviewed and did not reveal any outstanding discrepancies, design, or quality concerns.
