Date of event requested but not provided.(b)(4).Event evaluation a review of the device history record and the instructions for use (ifu) was conducted during the investigation. The device was not returned to assist in the investigation. The batch record for lot 4774622 was reviewed. It was confirmed that the product was manufactured in february of 2014 with a two year shelf life expiring in february of 2016.There were no issues/non conformances generated during the manufacturing of this device. There is no evidence to suggest that the product was not manufactured to the correct specifications. It should be noted that prior to implanting cook's device the patient aneurysm had ruptured.Once an aneurysm ruptures, the chance for survival is greatly reduced.Although endovascular repair can sometimes be done for a leaking or bleeding aneurysm, as in this case, the internal bleeding caused by the rupture was too severe. Therefore, the adverse event related to this case was most likely related to the morbidity of the patient prior to the procedure rather than complications stemming from the use of the device. The root cause was linked to the procedure itself along with the patient's condition being related to the occurrence.
