Investigation summary: the event unit was returned to applied medical for evaluation.Upon visual inspection, engineering confirmed that there was a hole in the shaft of the catheter.Testing was performed on a representative sample in order to try and replicate the complainant's experience.Engineering was able to replicate the complainant's experience by bending the catheter body and inserting the guide wire against the inner wall of the catheter. Therefore, it is likely that the hole was caused by an instrument.Applied medical will continue to monitor its vigilance system for trends and take appropriate actions as necessary.
