Additional Manufacturer Narrative
Investigation summary: the event unit was returned for evaluation.Upon visual inspection, engineering noted that the bottle had dents, scratches and rust near the welding.Water could be heard inside the canister walls. The likely root cause of the water inside the canister walls is due to improper storage, cleaning or usage conditions. While the applied medical scope warmer bottle is designed to be re-usable, the life span of the device is dependent on the conditions the device experiences. The observed dents and scratches on the bottle could have allowed water into the walls during sterilization. The instructions for use (ifu) state, "handle with care.Product should be stored in a clean, cool, dry area away from chemical fumes." all applied medical scope warmers are 100% inspected prior to packaging. It is unknown what conditions the product experienced during and after shipping from applied medical. In accordance to 21 cfr 803.56, if we obtain additional information, which was not known or was not available when this report was submitted, then the supplemental report will be submitted to the fda.
