Additional Manufacturer Narrative
|
|
A review of the device history records was performed for the indicated packaging and component lots for any deviations related to the reported defect of the complaint. The review confirms that the lots met all material, assembly and performance specifications. The (b)(6) 2017 angiodynamics complaint report was reviewed for the mini sticks product family and the failure mode, "guidewire broke/migrated." no adverse trend was identified.A supplier corrective action request, (scar) was forwarded to angiodynamics guidewire supplier, neometrics, however no device sample was returned from the complaint reporter.A photo was provided, but the image resolution was of poor quality.Neometrics stated that the image of the guidewire was "consistent with an elongated coil when tensile force was applied to the wire, " however, without a sample they were unable to determine a definitive cause for the broken wire. They reviewed their manufacturing records and no non-conformances were observed which would have contributed to this event. The guidewire lots passed all in-process and final inspection which included tensile and o.D.Inspections.(b)(4).Device not returned to manufacturer.
