Event Description
The incorrect side was cemented into the knee.It was noticed in pacu.The left femur was implanted in the right knee.The patient was bought back to the operating room and he was revised.A stem prosthesis was put in place.
 
Manufacturer Narrative
An event regarding implanting the wrong side triathlon femoral component in the patient was reported.The event was not confirmed.Method and results: device evaluation and results: not performed as the device was not returned for inspection.Medical records received and evaluation: no medical records were received for review with a clinical consultant.Device history review indicated the devices accepted into final stock from the reported lot were free from discrepancies.Complaint history review: there has been no other event for the lot referenced.Conclusions: the event could not be confirmed nor the root cause could be determined because the devices were not returned for evaluation and insufficient medical information was provided.No further investigation is required at this time.If additional information becomes available to indicate further evaluation is warranted, this record will be reopened.
