Additional Manufacturer Narrative
Additional narrative: the complaint device was received and evaluated.Visual examination revealed that the tip of the reamer was broken off at the distal end of the shaft where the flute begins, confirming this complaint. The broken fragment was not returned. This failure can be attributed to the use of excess force during device use, which is a user technique issue.Additionally, visual examination revealed some corrosion around the laser markings. The sales rep indicated that this device was used at several different facilities and no information regarding their autoclave and cleaning parameters is available. Therefore, a root cause for the observed corrosion and the time at which this issue occurred cannot be determined.Review of the device history record indicated that this batch of product was processed without incident; therefore, there is no internally assignable cause for the reported issue.Review of the depuy synthes mitek complaints system revealed no other complaints of any type for this lot of devices released to distribution. At this time, no corrective action is required and no further action is warranted. However, depuy synthes mitek will continue to track any related complaints within this device family to monitor the extent to which this complaint is observed in the field.(b)(4).
