During subsequent follow-up with the customer, it was mentioned that the device was in use with an electric pen drive during the event. The actual device was returned for evaluation.Reliability engineering evaluated the device and observed that the device failed the test adapter test (faulty), there was corrosion on connector ends and there was a displaced pin on socket connector ( pushed in). Therefore, the reported condition was confirmed. The assignable root cause was determined to be due improper cleaning methods and improper handling, which are both classified as user error. If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
