Product development investigation has been completed for part# 357.58, lot# 4294502.A visual inspection, drawing review and device history records (dhr) review were performed as part of this investigation. The returned slide hammer was confirmed to have a broken handle. The handle is in two pieces. The head of the hammer has some wear consistent with it's intended function. No other issues were noted.Replication of the complaint condition is not applicable as the device is already broken. The complaint is confirmed.Appropriate drawings were reviewed during investigation.During the investigation no product design issues or discrepancies were observed that may have contributed to the complaint condition. The returned parts were determined to be suitable for the intended use when employed. No definitive root cause was able to be determined. The device is 16+ years old.It is likely that cumulative wear, and repeated use and sterilization cycles caused weakening of the handle over time.Device was used for treatment, not diagnosis. If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
