Manufacturer Narrative

(b)(4).Catalog #: igtcfs-65-jp-jug-tulip.Similar to device under 510(k) k090140.(b)(4).Summary of investigational findings: three minor kink appears on the sheath approx.17 cm from the fitting.Furthermore, two severe kink appears on the sheath respectively 19.5 cm and 22.5 cm from the fitting. The kink which are placed 19.5 cm from the fitting has a mark, probably added to the sheath by a primary filter leg. However, there are no perforation of the sheath.Under normal conditions the sheath is strong enough to accomplish the procedure, but it has been seen before that the sheath may kink if somehow exposed to excessive force because of tortuous anatomy.According to the description of the event resistance was met when introducer was advanced through the sheath but procedure was abandoned. No evidence to suggest that this device was not manufactured according to specifications.Cook medical will continue to monitor for similar events.
