(b)(4).Catalog #: igtcfs-65-jp-jug-tulip.Similar to device under 510(k) k090140.(b)(4).Summary of investigational findings: three minor kink appears on the sheath approx.17 cm from the fitting.Furthermore, two severe kink appears on the sheath respectively 19.5 cm and 22.5 cm from the fitting.The kink which are placed 19.5 cm from the fitting has a mark, probably added to the sheath by a primary filter leg.However, there are no perforation of the sheath.Under normal conditions the sheath is strong enough to accomplish the procedure, but it has been seen before that the sheath may kink if somehow exposed to excessive force because of tortuous anatomy.According to the description of the event resistance was met when introducer was advanced through the sheath but procedure was abandoned.No evidence to suggest that this device was not manufactured according to specifications.Cook medical will continue to monitor for similar events.
 
Event Description
Description of event according to initial reporter: the patient was an emergency outpatient.The sheath was advanced to the ivc.Right after the filter introduer was advanced into the sheath, resistance was encountered, and the resistance kept the filter introducer from advancing despite several attempts.Then, the whole delivery system was removed from the patient, and another filter was placed.Patient outcome: the patient is hospitalized.
