Event or Problem Description

|
It was reported that the patient was hospitalized for post cardiac arrest and required therapeutic hypothermia.A solex catheter was inserted into the right internal jugular vein. The catheter insertion was described as difficult but was inserted in one attempt by the physician. After inserting the dilator, the physician attempted to pass the catheter but met significant resistance. The physician stopped and pulled the catheter back. The catheter was removed and another solex 7 catheter was placed which was inserted without any issues. The cooling therapy was continued successfully. There were no patient sequaela reported.Per reporter, the physician indicated that the catheter looked as it normally prior to insertion.Nothing was done with the catheter prior to insertion. The physician did not think she reached the vessel with the catheter prior to reaching resistance and thought it was likely still subcutaneous when resistance was met.The staff checked the balloons after the fact and there was one that they believed looked different.
