The event was reported via mw5066797.No films were received that confirmed the event. The available information suggests a prior surgery had occurred and the procedure on (b)(6) 2016 was an extension of the prior construct; no details of the prior surgery are known.Circumstances further suggest the patient may have lost their balance and the impact separated the connector components.Status post-revision of the fractured connector has been reported to be good.Explanted device was discarded by the user facility. No further evaluation of this component can be performed.Review of manufacturing records of the material type indicate no non-conformances have been observed.Literature review: ".Potential risks identified with the use of this system, which may require additional surgery, include: bending, fracture or loosening of implant component(s)." ".Loads on the device produced by load bearing and by the patient's activity level will dictate the longevity of the implant." ".The patient should be made aware of the limitations of the implant and potential risks of the surgery. The patient should be instructed to limit postoperative activity, as this will reduce the risk of bent, broken or loose implant components. The patient must be made aware that implant components may bend, break or loosen even though restrictions in activity are followed." device discarded by user facility.
