(b)(4).During processing of this complaint, attempts were made to obtain complete event, patient and device information.Concomitant medical products: guide wire: radifocus.Guide catheter: terumo. The device was not returned for evaluation. There was no report of any leaks noted during preparation for use, suggesting that the balloon was not damaged prior to the procedure.Since it was reported that the patients anatomical conditions were mildly calcified and stenosed, it is likely that the patients anatomical conditions interacted with the balloon causing damage to the balloon which contributed to the reported material rupture as the balloon was being inflated.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents reported from this lot.The investigation determined the reported difficulties to be related to the patient anatomical conditions. There is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.
