Investigation - evaluation a review of the complaint history, device history record, documentation, instructions for use (ifu), specifications, quality control, functional testing and visual inspection of the returned device was conducted during the investigation. One advance 14lp low profile balloon catheter was returned for evaluation.Visual inspection of the device revealed the presence of biomatter on the balloon, but no visible damage was noted to the balloon or the catheter shaft.During table top test a syringe was attached to the hub and the balloon inflated. No leaks to the hub or balloon were noted. It is plausible that the leakage was related to attaching the injection device to the catheter during the procedure, but since the other device was not returned we cannot determine that with certainty.A document based investigation evaluation was also performed. There is no evidence to suggest the product was not made to specifications.Review of device history record shows no nonconforming events which could contribute to this failure mode.It should be noted there was one additional complaint for this lot number; however, the complaint was unrelated to this device failure. The device is shipped with an instruction for use (ifu) that state, "upon removal from package, inspect catheter to ensure no damage has occurred during shipping." based on the information provided and results of the investigation, a definitive root cause could not be determined. We will continue to monitor for similar complaints.Per the quality engineering risk assessment no further action is required.
