Additional Manufacturer Narrative
One sample device was returned. The sample was a stingray connector with a catheter. The black insertion marking was properly inserted into the shovel portion of the connector.A flow test was conducted using a syringe filled with water and the returned device.Upon injection, water began to dispel from the damaged section of the catheter, confirming the reported complaint. The catheter was examined using the ce-180 (ram optical measurement system).During examination, it was evident that the catheter had been damaged by an external force; i.E.The internal spring was bent and no longer uniform, and the outer catheter wall was compressed at the area of the damage. The reported complaint stated that the end-user turned on the pump at 10pm and noticed the leak.Based on all applicable evidence, it was concluded that the damage to the catheter occurred after the original placement by the physician, but before the patient turned on the pump. The inspections include visual as well as functional testing to ensure device efficacy.Although root cause cannot be confirmed, it's possibly due to inappropriate use all information reasonably known as of (b)(6) 2016 has been included in this health authority report.Should additional information be obtained, a follow-up health authority report will be provided. The information provided by halyard health represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to halyard health.Halyard health has no independent knowledge of the event reported but is relaying the information that was provided by the user facility where the incident occurred. This product incident is documented in the halyard health complaint database and identified as complaint (b)(4).
