Additional Manufacturer Narrative
|
|
One used portex® bivona® uncuffed pediatric tracheostomy tube was returned for investigation.A visual inspection was performed and a split was observed on the flange eyelets. The most probable root cause was determined to be that the eyelet was damaged during use by coming into contact with a sharp edge, or the neck flange had a shot or a cut and it wasn't detected during the assembly process. The complaint was confirmed.
