Product development investigation was completed.A visual inspection under 5x magnification, device history record (dhr) review and drawing review were performed as part of this investigation. This complaint is confirmed. The tang/tab that aligns with the implant (nail) has sheared off of the returned insertion handle. The tang/tab that sheared off at its base was not returned.Whether this complaint can be replicated at customer quality (cq) is not applicable for this complaint condition as the returned device is already broken. The returned percutaneous insertion handle is a reusable instrument available for use in the lateral entry femoral nail-ex instrument set 01.003.300 per product support material. The device is used to insert lateral entry femoral nails percutaneous.A dimensional inspection of the tab feature was not able to be performed because the tang sheared off at its base and was not returned.Drawing was reviewed during this investigation. No product design issues or discrepancies were observed. This investigation was unable to determine a root cause. It was determined that the malfunction occurred most likely due to application of excessive force while not seated securely in the nail causing overload to the distal tang/tab.Device was used for treatment, not diagnosis. If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
