Event or Problem Description
It was reported that during the procedure the guide catheter leaked. No consequences to the patient were reported.

Additional Manufacturer Narrative
Analysis of the returned device revealed that the guide catheter was found to be leaking between the hub and proximal shaft, indicating that the leak originated from the hub.The guider was kinked on its shaft at approximately 32.0cm and 88.5cm from its proximal end.The found catheter damage (guide catheter shaft kinked) does not relate to the reported guide catheter leak because the locations of the damages are not in the same location as the leak. However, it is unknown what caused the found guide catheter shaft kinked.Based on the information available and device analysis it is confirmed that the leak originated from the hub and not the catheter shaft. Therefore the reported issue of the guide catheter shaft leak was not confirmed. The manufacturer has reviewed all information and determined this event no longer meets the requirement of the reportable event for the device in question.
