A fully constrained wallflex esophageal stent and delivery system was received for analysis.Visual analysis of the returned device found the blue outer sheath was found to be slightly buckled/accordioned just distal to the deployment handle and the clear outer sheath was also kinked. In addition, the tip was detached from the device and was not returned. The inner shaft was noted to be retracted into the outer sheath.Functional evaluation found the stent was possible to deploy as received. There were no issues noted with the stent. The distal inner was microscopically inspected and there was evidence of adhesive coverage along the length of the distal inner shaft. The damages noted with the device were consistent with the application of excessive force during use.The investigation concluded that the observed failures are likely due to anatomical or procedural factors encountered during the procedure which limited the performance of the device. Therefore, the most probable root cause for this complaint is operational context.A labeling review was performed, and from the information available this device was used per the directions for use (dfu) / product label.A review of the device history record (dhr) revealed no non-conforming events or deviations.A search of the complaint database confirmed that no similar complaints exist for the specified lot.
