A visual examination of the returned device revealed that the sheath was kinked at 15cm from its distal end.The pull wire was found to be kinked at the same area of the sheath and broken at its distal end.The rest of the pull wire and the basket were not present and not returned for inspection.Boston scientific manufacturing processes include extensive inspections to ensure that all finished devices meet specifications. The failures found could have been generated by the manipulation of the device, the interaction with the scope, or the interaction with other devices. These types of failures are consistent with ones caused when the product is being pulled/pushed with excessive force. This may cause damage, deformities, or cause the device to break. Therefore, the most probable root cause for this event is ¿operational context¿, since it is most likely that due to anatomical and/or procedural factors encountered during the procedure, the performance of the device was limited.A review of the device history record (dhr) confirmed that the device met all manufacturing specifications.A search of the complaint database revealed that no similar complaints exist for the specified batch.A labeling review was performed and no anomaly was found.
