Event Description
It was reported that during a veterinary tibial plateau leveling osteotomy repair surgery, it was observed that the saw attachment device was not locking properly while in use.As a result, there was a slight delay in the surgical procedure while the technician retrieved a spare device.It was reported that the case was successfully completed.There was no human patient involvement as this was a veterinary procedure.There were no reports of injuries, medical intervention or prolonged hospitalization.All available information has been disclosed.If additional information should become available, a supplemental medwatch report will be submitted accordingly.
 
Manufacturer Narrative
Correction: the date the device was returned to the manufacturer was updated to june 15, 2017.This device was returned for service; however, did not meet manufacturing specifications during pre-repair assessment.During repair, it was determined that the housing was loose (adhesive between the housing and the coupling was loose) and the frequency could not be measured.Therefore, the reported condition was confirmed.The assignable root cause was determined to be due to wear from normal use and servicing.If additional information should become available, a supplemental medwatch will be submitted accordingly.
