Additional Manufacturer Narrative
This report is being submitted as follow up no.1 to provide the device return date & the evaluation results. The actual sample was returned to the manufacturing facility for evaluation.Visual inspection upon receipt confirmed the customer's complaint. The actual sample had been snapped off at approximately 150mm from the distal end of the device.Magnifying inspection of the fracture on the distal side found that the edge of the fractured shaft had been stretched and deformed into a flaring shape with the segment adjacent to the fracture having been deformed into an accordion-like shape. No other damage, such as a scratch was not found around the fracture.Magnifying inspection of the fracture on the proximal side found that the braided wire (stainless steel reinforcement) had been exposed at the fracture end.No other damage, such as a scratch was not found around the fracture.Magnifying inspection of the rest of the device did not find any anomaly, including a deformity or scratch. The inside and outside diameters were measured and confirmed to meet manufacturer specifications. The bending resistance was determined and confirmed to be within the specifications, being comparable to that of the current product sample. There is no evidence that this event was related to a device defect or malfunction.Although the exact cause of the reported event cannot be definitively determined based on the investigation, it is likely that the distal end of the actual sample was trapped due to some factor(s) and, in this state, it was subjected to some torque force. The torque force was concentrated to the segment where the braiding ends (where the physical properties of the shaft transfer from "braided" to "non-braided" and the shaft was distorted. The guide wire used in combination with the actual sample was caught in this distorted segment.Subsequently, when the actual sample was withdrawn, it got snapped at the point where the guide wire was trapped with it.All available information has been placed on file in quality assurance for appropriate tracking, trending and follow-up.
