Catalog Number 42965
Medical Device Problem Code Occlusion Within Device (1423)
Health Effect - Clinical Codes Therapeutic Response, Decreased (2271); Hydrocephalus (3272)
Date of Event 05/04/2017
Type of Reportable Event Serious Injury
Additional Manufacturer Narrative
The returned valve was patent. The valve met the requirements for siphon, and preimplantation testing. However, the device did not meet requirements for reflux, and pressure flow testing. There was proteinaceous debris observed within the interior and exterior of the valve.Debris within the valve may hold pressure-flow controlling mechanisms open resulting in fluid reflux and affecting the flow of fluid through the valve. The instructions for use that accompany the device caution that ¿shunt obstruction may occur in any of the components of the shunt system. The system may become occluded internally due to tissue fragments, blood clots, tumor cell aggregates, bacterial colonization, or other debris.¿ the returned valve did not meet the requirements for leak testing due a tear observed in the top of the delta chamber. It is unknown how or when this damage occurred. The ifu cautions that ¿care should be taken in handling the valves as silicone has a low cut and tear resistance.¿ the ifu also cautions that ¿improper use of instruments in the handling or implantation of shunt products may result in the cutting, slitting, crushing or breaking of components.¿ a review of the manufacturing records showed no anomalies. All valves are 100% tested at the time of manufacture. If information is provided in the future, a supplemental report will be issued.

Event or Problem Description
It was reported to medtronic neurosurgery that the patient presented with worsening hydrocephalus symptoms, indicating the ventricular peritoneal shunt was not functioning correctly.According to the report, the patient was experiencing variable conscious states elevating their risk of aspiration. The physician attempted to troubleshoot by reducing the performance level to 0.5.Reportedly, the status of the patient did not change and the physician elected to take the patient back to theatre and revise the shunt. The product was replaced and it was stated the physician believed the cause was intermittent shunt blockage, though no specific component was identified. It was added that the valve was bathed in the standard ringer¿s solution prior to implant and there were no mri¿s performed while the shunt was implanted.
