Correction: the sample was returned and evaluated. One used sample was received with the product packaging. The sample was received with significant buildup inside the tubing. The cuff exhibits tearing from just below the base of the proximal collar to the medial point. At that point, the tear continues radially approximately two-thirds around the cuff. The cuff material was pressed back into position. There are no visible missing pieces -all of the cuff appears present. Both the proximal and distal collars remain attached to the tubing. No visible point of origin for the tear was observed. The leak could probably have been due to the tearing of the cuff. The device history record for the lot number, um6077, involved in this complaint was reviewed and the material was produced according to the manufacturing procedures and met the quality requirements.A root cause could not be determined. All information reasonably known as of 10aug2017 has been included in this health authority report.Should additional information be obtained, a follow-up health authority report will be provided. The information provided by halyard health represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to halyard health.Halyard health has no independent knowledge of the event reported but is relaying the information that was provided by the user facility where the incident occurred. This product incident is documented in the halyard health complaint database and identified as complaint (b)(4).
