Event or Problem Description
The customer's parent reported via phone call that the insulin pump was not functioning properly. They were changing the infusion set when the insulin pump alarmed a motor error and all of the settings were erased. The customer's blood glucose level was 536 mg/dl.The alarm history was reviewed and they found multiple no delivery alarms, multiple motor error alarms, and one motor position encoder error alarm. The customer was advised to discontinue use of the device and revert to a back-up plan. The insulin pump was returned for analysis.

Additional Manufacturer Narrative
The insulin pump alarmed motor error during the basic occlusion test due to faulty force sensor resistor. The motor was tested outside of the device and passed. We were unable to confirm encoder signal alarm due to overwritten history file and unable to confirm excessive no delivery alarms due to motor error alarm. The insulin pump passed displacement test, self-test and unexpected restart error test. The insulin pump passed all operating currents tested within the specification range and passed off no power test.No unexpected resetting or settings anomaly noted. The insulin pump was received with broken battery tube thread, cracked reservoir tube lip, cracked reservoir tube, cracked reservoir tube window, cracked case near display window corners, missing end cap sticker and minor scratches on display window noted. The drive support cap was inspected and no anomaly was noted.
