Manufacturing review: the device history records were reviewed with special attention to the raw materials, subassemblies, manufacturing process and quality control testing. This lot met all release criteria. There was nothing found to indicate there was a manufacturing related cause for this event. This is the only complaint reported to date for this lot number and failure mode.Visual/microscopic inspection: the balloon size for this product is printed on the balloon hub of the catheter and identified the returned sample as a 20mm x 4.5cm balloon. The balloon was examined under microscopic magnification (30x), and frayed fibers were noted on the distal cone of the balloon. No other anomalies were identified on the device.Functional/performance evaluation: the patency of the guidewire lumen was tested using an in-house 0.035" guidewire and it passed without issue. The device was connected to an in-house inflation device and the balloon was inflated to nominal pressure and rated burst pressure (rbp) without issues. The balloon was then fully deflated without issue.Medical records review: medical records were not provided; therefore, a review could not be performed.Image/photo review: images/photos were not provided; therefore, a review could not be performed. Conclusion: a visual evaluation found frayed fibers on the distal cone of the balloon. Therefore, the investigation is confirmed for frayed material.Based on limited details from the complainant, it is unknown if the frayed material was the alleged issue. Therefore the investigation is inconclusive for the reported device malfunction. The definitive root cause for the frayed fibers could not be determined based upon the available information. It is unknown if patient and/or procedural issues contributed to the reported event.Labeling review: the current ifu (instructions for use) states: precautions: carefully inspect the catheter prior to use to verify that catheter has not been damaged during shipment and that its size, shape and condition are suitable for the procedure for which it is to be used.Do not use if product damage is evident. The minimal acceptable french size is printed on the package label.Do not attempt to pass the catheter through a smaller size sheath introducer than indicated on the label. If resistance is felt during post procedure withdrawal of the catheter through the introducer sheath, determine if contrast is trapped in the balloon with fluoroscopy. If contrast is present, push the balloon out of the sheath and then completely evacuate the contrast before proceeding to withdraw the balloon. If resistance is still felt during post procedure withdrawal of the catheter, it is recommended to remove the balloon catheter and guidewire/introducer sheath as a single unit.In order to activate the hydrophilic coating, it is recommended to wet the ultraverse catheter with sterile saline solution immediately prior to its insertion in the body.Dilatation catheter preparation: remove catheter from package.Verify the balloon size is suitable for the procedure and the selected accessories accommodate the catheter as labeled.Remove the balloon guard and stylet by grasping the balloon catheter just proximal to the balloon and with the other hand, gently grasp the balloon protector and slide distally off of the balloon catheter. In order to activate the coating, wet the balloon catheter with sterile saline immediately prior to its insertion into the introducer sheath.
