Evaluation summary: post market vigilance (pmv) led an evaluation of five devices. The visual inspection of the returned product noted that on device one there were gouges at the distal end of the trocar. The tip of the white retractable sheath was broken off.The visual inspection of the other four devices noted that they were received intact in the appropriate packaging.Pmv performed functional testing of device one: the device passed an air leak test.Pmv performed functional testing of the other four devices; the devices were removed from their packaging, they passed an air leak test and all other components functioned properly.Records from each manufacturing lot are thoroughly reviewed to ensure that products are released meeting all quality release specifications at the time of manufacture.Analysis concluded there were no assembly component related failures.Replication of the gouges at the distal end of the trocar and the broken tip of the white retractable sheath may occur when the devices are mishandled during clinical application.Based on the evidence available the reported condition was confirmed.Should new information become available, the file will be re-opened and the investigation summary will be amended as appropriate. If information is provided in the future, a supplemental report will be issued.
