Additional Manufacturer Narrative
The following were reviewed as part of this investigation: patient severity, frequency analysis, applicable previous investigation(s), sample (if available), applicable fmea documents, labeling, and applicable manufacture records.Based on a review of this information, the following was concluded: the complaint of a break in the extension set tubing was confirmed, but the exact cause was not determined since the complete sample was not returned for investigation. One extension set with purple clamp from a powerglide pro kit was returned for investigation. The returned sample exhibited evidence of use.Blood residue was observed within the lumen.Reside from a dressing remained adhered to the external surface of the tubing. The female luer was not returned for investigation. The adhesive fillet was observed around the tube where the luer adaptor would have been located, which indicates that the product was manufactured correctly. The extension tubing extended 1.5mm from the adhesive fillet. The end of the tubing that extended from the fillet exhibited a rough and jagged edge and surface. It appears that the extension tube fractured within the luer adaptor. The proximal end of tubing may still reside within the luer adaptor.Since the break was located within the luer adaptor, the extension set tubing may have been subject to tensile, and possibly torsional, stresses that exceeded the strength of the extension set tubing. This can occur with patient movement/manipulation or inadvertent pulling on the extension set.Excessive force applied to the extension set tubing or luer adaptor should be avoided during use.A lot history review (lhr) of rebq1331 showed no other similar product complaint(s) from this lot number.
