Event or Problem Description
|
|
As reported by a healthcare professional, during coil embolization of an unruptured aneurysm, there was resistance between a deltaxtrasoft (dlx100254, s11984) and a headway 17 microcatheter, and during coil resheathing, the sheath introducer was damaged and the coil could not be re-sheathed.The micro catheter was approached to the lesion as steam-shaped.¿the only inflow got clogged tightly when the coils (micrusframes5x17, micruframec4x8, deltaxsft3x6, deltaxsft3x6, deltaxsft2.5x4) was inserted¿ , so the physician tried to re-sheath the deltaxtrasoft (complaint product), but the sheath introducer got damaged and could not be re-sheathed.After that, the micro catheter was reinserted, and the coil was replaced with another one (same size) and additional 6 coils were placed in the lesion. The procedure was successfully completed without further issues or delay. There was also no patient injury / complication reported. The complaint product was new and stored per labeling instructions. The procedure was conducted in accordance with the ifu and the constant flush had been maintained at all time.No visible defect/damage was noted on the products prior to the event. No unintended detachment was observed in the vessel or in the microcatheter. The product was not available for the investigation. No further information was available.
