As the reported defective device was not returned, angiodynamics is unable to perform a device evaluation. The customer's reported complaint description of guidewire coming apart cannot be confirmed as the sample was not returned for evaluation.Without doing a device evaluation, a definitive root cause for the reported complaint description cannot be determined.A review of the lot history records was performed for the reported packaging and incoming component lot for any deviations related to the reported defect of the complaint. The review confirms that the lots met all material, assembly, and performance specifications. The guidewire is a purchased component.Angiodynamics' vendor was made aware of this complaint per (b)(4).As stated in the vendor's response: "although the exact root cause of the event is unable to be determined, it does not appear to be the result of manufacturing".The instructions for use, which is supplied to the end user with this catalog number instructs the user to withdraw the needling while leaving the guidewire in place.A review of the angiodynamics complaint system noted no trends for this complaint type and product family. This type of complaint will continue to be monitored for trends.Complaint # (b)(4).
