Additional Manufacturer Narrative

Evaluation summary: post market vigilance (pmv) led an evaluation of two devices.Visual evaluation of both the videos and received samples found no abnormalities which would indicate any manufacturing issue which may have contributed to this failure.Evidence of material transfer as a result of the welding process was found on each specimen; however, it is not possible to verify the tensile load that was applied to the opened suture received which caused it to break.Additionally, a review of the video evidence provided found that the suture was not applied in accordance with the ifu, as the surgeon took several extra bites before applying tension to appose the tissue and anchor the loop.A review of the device history record indicates the product was released meeting all quality release specifications at the time of manufacture. The root cause of the observed damage was misuse of the product which would have caused or contributed to the reported incident.Should new information become available, the file will be re-opened and the investigation summary will be amended as appropriate.
