(b)(4).One 6f adelante sigma introducer sheath from lot# dp-05207 was returned from the customer. There were no other accessories.Traces of blood were found on and inside the sheath. The distal tip of the sheath was cut off and the usable length is only 90mm long. There is an area on the sheath tube near the hub that had an accordion effect near the hub that is approximately 250mm long.Returned device analysis reveals one 6f adelante sigma introducer sheath that incurred physical damage during use out in the field.Per drawing, the usable sheath length after tipping should be 100mm ± 2mm.No manufacturing defects were found.Based on the investigation, a capa is not required as findings did not identify a design, labeling or manufacturing non-conformity.In addition, there was no new failure mode identified and the risk remains acceptable. The event will be re-evaluated if additional information becomes available.Oscor will continue to monitor this event type and risk.Per procedure (adelante sigma sheath in-process and final inspection), aql inspection level: verify the snap lock bonded cap is properly assembled and aligned on sheath hub.Gently pull on cap and rotate to ensure it is firmly attached to sheath hub.Under 10x magnification, verify the tip is round, no flash, burnt material, no dents.Verify proper tip shape.Using a ruler, measure the length from end of hub to sheath tip per part drawing.Ensure parts are free of loose or embedded foreign matter.Look for any damages such as flash, dents, melted/burnt material or excessive adhesive in cap and sideport joints.Per instructions for use (ifu): precautions/contraindications: manipulate the guidewire slowly and carefully, not to damage the vessel wall, while monitoring the tip position and movement under fluoroscopy.
