This mdr is being filed after the associated complaint was reviewed under remediation protocol (b)(4), complaint/mdr retrospective review and remediation and reassessed as reportable.Additional complaint investigation and record remediation was not performed.
Event or Problem Description
It was reported that prior to an unspecified procedure, the tip of the performer introducer was noted to be melted flat, with no opening.As a result, another device was used for the procedure.Device evaluation identified /clarified there was nick type damage and flattening at the dilator tip.
