Event Description
|
It was reported that the procedure was to treat an in-stent restenosis with heavy tortuosity and heavy calcification in the mid circumflex coronary artery.The 3.5 x 12 mm nc trek was advanced easily to the target lesion and inflation was performed.However, when trying to remove the deflated balloon, there was resistance and the balloon got stuck somewhere in the vessel.When the device was pulled out, a piece of the balloon was left inside the patients anatomy.The patient was sent for coronary artery bypass (cabg) surgery.No additional information was provided.

(b)(4).Evaluation summary: visual inspection was performed on the returned device.The reported separation was confirmed; however, the reported difficulty removing the device from the anatomy could not be replicated in a testing environment as it was based on operational circumstances.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents from this lot.The investigation determined the reported difficulties appear to be related to circumstances of the procedure.There is no indication of a product quality issue with respects to the design, manufacture, or labeling of the device.
