Customer quality (cq) engineering investigation: visual inspection: opal implant holder (p/n 03.803.002) was returned at cq location with the handle in a loose condition. The handle was found to be displaced downwards from its ideal position hence is wobbling. This will interfere with the proper functioning of the device.Knob and shaft were not returned with the complaint. The balance of the returned device is in slightly worn condition with superficial wear marks all over the device surface. The device is approx.9 years old.The complaint condition is most probably a result of cumulative wear of the device during such a long time of usage combined with a possible malhandling of the device either during surgery or sterilization process.Replication of the complaint condition could not be performed at cq location due to confirmed damage to the device. No other damage was observed.Visual inspection, device history record (dhr) review, service & repair evaluation and drawing review were performed as part of this investigation. This complaint is confirmed.Device used for treatment, not diagnosis. If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
