Event Description
It was reported that while the lumbar catheter was inserted and was directed towards the spinal canal, it seem that the tuohy needle tip and the catheter tip were unable to be advanced due to being caught.Only the catheter was attempted to be removed, although the tuohy needle should have been removed as well.Then, the catheter fractured and a piece of it remained in the patient body.

Additional information received reported that the physician determined that the broken piece left inside the patient did not pose a risk to the patient and would not be likely to cause or contribute to a death or serious injury.
