Note: this is a remediation mdr. Medtronic diabetes implemented revised mdr reportability criteria effective on (b)(6). Subsequently, medtronic diabetes conducted a two year retrospective review of complaints. This event was retrospectively identified to be reportable based on the revised mdr reportability criteria. Currently it is unknown whether or not the device may have caused or contributed to the event as no product has been returned. No conclusion can be drawn at this time. We therefore consider this report complete to the best of our knowledge.
 
Event Description
Customer's grandfather is reporting that the customer is experiencing an occlusion with her insulin delivery. Customer reports that her pump and infusion set would deliver insulin before insulin delivery would stop. Customer's grandfather is able to troubleshoot. Customer's grandfather reports that there is blood at the site of the insertion. Customer's grandfather reports that upon removal, the infusion set cannula was bent and had blood on it. Customer's blood glucose was 183 mg/dl at the time of the incident. The product is not expected to be returned.
