Event or Problem Description
|
|
Medtronic received information that during the implant of this transcatheter bioprosthetic valve, the valve was fully deployed and the delivery catheter system (dcs) was removed. The non-medtronic guidewire was then removed from the ventricle and caught on the inflow portion of the valve frame, causing the valve to then dislodged into the ascending aorta.Subsequently, a second transcatheter bioprosthetic valve was implanted. No adverse patient effects were reported.
