Additional Manufacturer Narrative
The following were reviewed as part of this investigation: patient severity, frequency analysis, applicable previous investigation(s), sample (if available), applicable fmea documents, labeling, and applicable manufacture records.Based on a review of this information, the following was concluded: the complaint of a break is confirmed and was determined to be use related. One 5 fr single lumen power groshong was returned for evaluation. An initial visual observation showed the catheter was returned in two sections and was observed to be broken between the 39 cm and 40 cm depth markers.Use residue was observed throughout the sample. Some tensile weakness was observed in the distal segment of the catheter approximately 3.2 cm proximal to the distal tip.A microscopic observation revealed the fracture was angular in shape and, on both fracture surfaces, it was observed that approximately one half of the surface was smooth and rounded and the other half was granular in texture. The surface of the catheter near the smooth portions of the fracture sites was observed to also be smooth and appeared to be polished. The polished and rounded sections of the break surface indicate flexural fatigue of the catheter and the granular sections of the break surface indicate a tensile break, which most-likely occurred during catheter removal due to the weakened catheter material at the site of the flexural fatigue.A lot history review (lhr) of reaz0732 showed one other similar product complaint(s) from this lot number. The complaints for this lot number (reaz0732) have been reported from the same facility.
