Event Description
According to the reporter, during laparoscopic intra peritoneal onlay mesh hernia repair, the surgeon was tacking mesh into place and noticed two of four transfacial sutures were not holding on to mesh, as a result he finished tacking and opened a site above the defect to close the defect.The mesh was cut prior to implantation and 4 fixation points were applied.The mesh had a normal appearance.A 5mm trocar was used.There was an additional incision made to close the defect above the hernia, the defect was closed with a suture and case was finished.There was no patient injury.
